Not got the tab, but the basics are quite easy.
Open D tuning, straight 12 bar but holding the A chord for bars 9 and 10.
Bass is alternating between the 6th and 4th string Ds
Over the D chord try something like (with the slide on the top D):

2 slide up to 4------------------- 2 slide up to 4----2 pull off to 0---2 slide up to 4

On the change to the G chord use the open position (low to high):

x 2 0 1 0 0

Walk up to this chord on the 5th string with the first beat of the bar on 2:
x-x|x-x-x-x-x-x-x-x|4----
x-x|x-x-x-x-x-x-x-x|x-x-x
x-x|x-1-x-1-x-1-x-x|x-x-x
x-x|x-0-x-0-x-0-x-3|x-0-x
0-1|2-x-2-x-2-x-2-x|x-x-x
x-x|x-x-x-x-x-x-x-x|0-x-0 etc

On the A chord keep the alternating bass on the 6th and 4th string Ds and play on the top string with slide (tabbed in approx 1/8th notes):

2-x-x-2-x-x-x-x-4-x-4-x-2-0-
x-x-etc
x-x-etc
x-x-etc
x-x-0-x-x-x-0-x-x-x-0-x-x-x-0-x-x-x-0-x
0-x-x-x-0-x-x-x-0-x-x-x-0-x-x-x-0-x-x-x

The last bit is just a hammer on on the second string second fret followed by the open 1st string while keeping the alternating bass, syncopate it how you like and vary it over the verses.

Hope this helps.
